Joining the Form was the best decision I’ve made in my life. After joining I was able to speak with Dr Smith about my life and struggles. Dr Smith listened and asked questions to really understand me and what I’ve been through in my life to come up with a solution. My dietitian Alysha helped me with my diet, proteins, exercise. Things really worked and I proudly can say I’m 86.6 pounds lighter. I’m still striving to my goals.
