The forms on pdf would be much more beneficial and look more professional if the forms' areas for input could vary with required data input.
There were also 2 typographical errors in the pdf that cannot be corrected.
Probably a word document type file would be best.

They offer "Printed" forms for $70 but there is a very poor explanation of what is required or provided.
I will probably use different vendor in the future.
