I returned one of two cards I paid for by USPS and they returned with no address. The card had one and the prescription had another unknown address. I am out 85 dollars 5

"I haven’t got my card yet"

I haven’t got my card yet. When I filled out the Doctors form I didn’t know I was suppose to fill 1 out to the State. The doctor must have told me but am almost deaf even with hearing aids plus it’s hard for me to understand some folks voices to others. When I did fill out the State form I was sure I added my social security number but was told I didn’t plus I marked the wrong thing on the form for qualification. I resubmitted the State form correcting my mistakes. I hope I got it right this time and qualify for my medical card. Thanks
